(Nejedn\u00e1 se o popis v\u00fdrobku, z\u0159eknut\u00ed se odpov\u011bdnosti je uvedeno na konci str\u00e1nky)<\/h2>\n<p>Ivermektin na Demodex (nu\u017ceniec) je st\u00e1le \u010dast\u011bji diskutov\u00e1n jako jedna z metod podporuj\u00edc\u00edch boj s ko\u017en\u00edmi probl\u00e9my spojen\u00fdmi s p\u0159\u00edtomnost\u00ed rozto\u010d\u016f Demodex. V posledn\u00edch letech z\u00e1jem o tuto slou\u010deninu v\u00fdrazn\u011b vzrostl, zejm\u00e9na v kontextu v\u00fdzkumu jej\u00edho mechanismu \u00fa\u010dinku a pozorov\u00e1n\u00ed u\u017eivatel\u016f.<\/p>\n<p>V k\u016f\u017ei se nach\u00e1z\u00ed mnoho mikroorganism\u016f, kter\u00e9 obvykle \u017eij\u00ed v souladu s organismem. Mezi n\u011b pat\u0159\u00ed rozto\u010di <i>Demodex v Polsku zn\u00e1m\u00fd jako <strong>cysticerk\u00f3za<\/strong><\/i>, mikroskopick\u00e9 organismy ob\u00fdvaj\u00edc\u00ed vlasov\u00e9 folikuly a mazov\u00e9 \u017el\u00e1zy. U \u010dlov\u011bka se vyskytuj\u00ed dva hlavn\u00ed druhy: <i>Demodex folliculorum <\/i>i <i>Demodex brevis<\/i>. A\u010dkoli jsou obecn\u011b ne\u0161kodn\u00e9, jejich nekontrolovan\u00e9 p\u0159emno\u017een\u00ed m\u016f\u017ee v\u00e9st ke ko\u017en\u00edm a o\u010dn\u00edm probl\u00e9m\u016fm. <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1367048419300530\" target=\"_blank\" rel=\"noopener\"><sup>(1)<\/sup><\/a><\/p>\n<p>P\u0159elidn\u011bn\u00ed <i>Demodex <\/i>je spojov\u00e1n s onemocn\u011bn\u00edmi, jako je r\u016f\u017eovka, demodekt\u00f3za, z\u00e1n\u011bt o\u010dn\u00edch v\u00ed\u010dek a keratitida. P\u0159\u00edznaky mohou zahrnovat sv\u011bd\u011bn\u00ed, zarudnut\u00ed, papuly, pustuly nebo \u0161upinat\u00e1 v\u00ed\u010dka, \u010dasto p\u0159ipom\u00ednaj\u00edc\u00ed akn\u00e9 nebo dermatitidu. Kv\u016fli t\u011bmto podobnostem se diagn\u00f3za \u010dasto odkl\u00e1d\u00e1, co\u017e prodlu\u017euje pot\u00ed\u017ee a komplikuje l\u00e9\u010dbu. Star\u0161\u00ed lid\u00e9, pacienti s oslaben\u00fdm imunitn\u00edm syst\u00e9mem nebo pacienti s mastnou k\u016f\u017e\u00ed maj\u00ed tendenci k vy\u0161\u0161\u00edmu v\u00fdskytu infekce, co\u017e poukazuje na roli faktor\u016f souvisej\u00edc\u00edch s hostitelem p\u0159i vzniku onemocn\u011bn\u00ed. Tradi\u010dn\u00ed l\u00e9\u010dba - nap\u0159. permethrin, metronidazol a sirn\u00e9 p\u0159\u00edpravky - m\u016f\u017ee sn\u00ed\u017eit po\u010det rozto\u010d\u016f, ale \u010dasto je omezena ne\u00fapln\u00fdm odstran\u011bn\u00edm, recidivou a podr\u00e1\u017ed\u011bn\u00edm. Tyto nedostatky vedly ke zv\u00fd\u0161en\u00e9mu z\u00e1jmu o nov\u011bj\u0161\u00ed terapie, kter\u00e9 jsou \u00fa\u010dinn\u00e9 a l\u00e9pe sn\u00e1\u0161en\u00e9. Topick\u00fd ivermektin 1% je ji\u017e schv\u00e1len pro l\u00e9\u010dbu rosacey, zat\u00edmco peror\u00e1ln\u00ed formy jsou zkoum\u00e1ny pro \u0161ir\u0161\u00ed pou\u017eit\u00ed u onemocn\u011bn\u00ed souvisej\u00edc\u00edch s rozto\u010di. Tento \u010dl\u00e1nek pojedn\u00e1v\u00e1 o \u00faloze ivermektinu p\u0159i kontrole <i>Demodex, <\/i>jej\u00ed v\u00fdhody oproti konven\u010dn\u00edm l\u00e9\u010debn\u00fdm postup\u016fm a jej\u00ed potenci\u00e1l jako prim\u00e1rn\u00ed terapie pro zlep\u0161en\u00ed zdrav\u00ed k\u016f\u017ee a kvality \u017eivota. Nejzn\u00e1m\u011bj\u0161\u00edm z nich je rosacea, u n\u00ed\u017e pacienti trvale vykazuj\u00ed vy\u0161\u0161\u00ed hustotu rozto\u010d\u016f ve srovn\u00e1n\u00ed se zdrav\u00fdmi jedinci. Metaanal\u00fdza publikovan\u00e1 v \u010dasopise <i>Journal of the American Academy of Dermatology (2012) <\/i>uk\u00e1zala, \u017ee lid\u00e9 s r\u016f\u017eovkou maj\u00ed t\u00e9m\u011b\u0159 osmkr\u00e1t vy\u0161\u0161\u00ed pravd\u011bpodobnost, \u017ee budou infikov\u00e1ni<i> Demodex<\/i>, zejm\u00e9na u papulopustul\u00e1rn\u00edch podtyp\u016f. V oftalmologick\u00fdch studi\u00edch byla dob\u0159e zdokumentov\u00e1na demodexov\u00e1 eyeliditida, chronick\u00fd z\u00e1n\u011bt o\u010dn\u00edch v\u00ed\u010dek. V jedn\u00e9 studii bylo zji\u0161t\u011bno, \u017ee t\u00e9m\u011b\u0159 84% pacient\u016f s chronick\u00fdm z\u00e1n\u011btem o\u010dn\u00edch v\u00ed\u010dek m\u011blo rozto\u010de <i>Demodex<\/i>. Mezi klinick\u00e9 p\u0159\u00edznaky pat\u0159\u00ed v\u00e1lcovit\u00e9 lupy na b\u00e1zi \u0159as, kter\u00e9 jsou spolehliv\u00fdm indik\u00e1torem napaden\u00ed.<\/p>\n<p>Nel\u00e9\u010den\u00e1 infekce m\u016f\u017ee v\u00e9st k poru\u0161e funkce Meibomovy \u017el\u00e1zy a p\u0159isp\u011bt k rozvoji syndromu such\u00e9ho oka. V\u00fdzkum publikovan\u00fd v \u010dasopise <i>Investigative Ophthalmology &amp; Visual Science (2011) <\/i>potvrdil, \u017ee hustota rozto\u010d\u016f koreluje jak s blok\u00e1dou Meibomovy \u017el\u00e1zy, tak se z\u00e1va\u017enost\u00ed p\u0159\u00edznak\u016f syndromu such\u00e9ho oka. Ve vz\u00e1cn\u00fdch, ale z\u00e1va\u017en\u00fdch p\u0159\u00edpadech m\u016f\u017ee napaden\u00ed zp\u016fsobit keratitidu, kter\u00e1 hroz\u00ed ztr\u00e1tou zraku. Proto\u017ee se p\u0159\u00edznaky - p\u00e1len\u00ed, sv\u011bd\u011bn\u00ed a pocit ciz\u00edho t\u011blesa - p\u0159ekr\u00fdvaj\u00ed s p\u0159\u00edznaky alergick\u00e9 konjunktivitidy a syndromu such\u00e9ho oka, diagn\u00f3za se \u010dasto odkl\u00e1d\u00e1. Star\u0161\u00ed lid\u00e9 maj\u00ed vy\u0161\u0161\u00ed incidenci napaden\u00ed rozto\u010di; jedna \u010d\u00ednsk\u00e1 kohortov\u00e1 studie uv\u00e1d\u00ed incidenci vy\u0161\u0161\u00ed ne\u017e 80% u lid\u00ed star\u0161\u00edch 60 let ve srovn\u00e1n\u00ed s m\u00e9n\u011b ne\u017e 30% u lid\u00ed mlad\u0161\u00edch 20 let. S\u00e9rie p\u0159\u00edpad\u016f popisuje fulminantn\u00ed demodekt\u00f3zu obli\u010deje u imunokompromitovan\u00fdch pacient\u016f, kter\u00e1 ustoupila a\u017e po c\u00edlen\u00e9 akaricidn\u00ed terapii. Mezi dal\u0161\u00ed rizikov\u00e9 faktory pat\u0159\u00ed mastn\u00e1 ple\u0165, chronick\u00e9 u\u017e\u00edv\u00e1n\u00ed steroid\u016f a sou\u010dasn\u00fd v\u00fdskyt seboroick\u00e9 nebo atopick\u00e9 dermatitidy, kter\u00e9 vytv\u00e1\u0159ej\u00ed p\u0159\u00edzniv\u00e9 podm\u00ednky pro mno\u017een\u00ed rozto\u010d\u016f. P\u0159\u00edznaky, jako je zarudnut\u00ed, sv\u011bd\u011bn\u00ed nebo pup\u00ednky, jsou spole\u010dn\u00e9 pro acne vulgaris, seboroickou dermatitidu a rosaceu. U o\u010dn\u00edch onemocn\u011bn\u00ed je diagnostika komplikov\u00e1na p\u0159ekr\u00fdv\u00e1n\u00edm p\u0159\u00edznak\u016f alergick\u00e9 konjunktivitidy a syndromu such\u00e9ho oka. Zlat\u00fdm standardem pro diagnostiku z\u016fst\u00e1v\u00e1 p\u0159\u00edm\u00e9 pozorov\u00e1n\u00ed pod sv\u011bteln\u00fdm mikroskopem. Obvykle se p\u0159i hled\u00e1n\u00ed rozto\u010d\u016f vy\u0161et\u0159uj\u00ed ko\u017en\u00ed se\u0161kraby nebo vytrhan\u00e9 \u0159asy. Nov\u011bj\u0161\u00ed metody, jako je konfok\u00e1ln\u00ed mikroskopie, v\u0161ak umo\u017e\u0148uj\u00ed zobrazit rozto\u010de ve vlasov\u00fdch folikulech in vivo, co\u017e zvy\u0161uje p\u0159esnost diagnostiky. (<a href=\"https:\/\/journals.lww.com\/ijd\/fulltext\/2012\/57010\/facial_demodicidosis__a_diagnostic_challenge.22.aspx\" target=\"_blank\" rel=\"noopener\"><sup>7)<\/sup><\/a><\/p>\n<h2>Tradi\u010dn\u00ed p\u0159\u00edstupy a jejich omezen\u00ed<\/h2>\n<h3>Lok\u00e1ln\u00ed l\u00e9\u010dba: metronidazol a permethrin<\/h3>\n<p>Po mnoho let byly nejb\u011b\u017en\u011bj\u0161\u00ed l\u00e9\u010dbou ko\u017en\u00edch onemocn\u011bn\u00ed zp\u016fsoben\u00fdch Demodexem lok\u00e1ln\u00ed p\u0159\u00edpravky, jako je metronidazol a permethrin. Metronidazol je \u0161iroce pou\u017e\u00edv\u00e1n p\u0159i l\u00e9\u010db\u011b rosacey d\u00edky sv\u00e9mu protiz\u00e1n\u011btliv\u00e9mu \u00fa\u010dinku, ale jeho schopnost sn\u00ed\u017eit po\u010det rozto\u010d\u016f je omezen\u00e1. To znamen\u00e1, \u017ee u pacient\u016f m\u016f\u017ee doj\u00edt ke zlep\u0161en\u00ed zarudnut\u00ed a podr\u00e1\u017ed\u011bn\u00ed, zat\u00edmco populace rozto\u010d\u016f z\u016fst\u00e1v\u00e1 nezm\u011bn\u011bna, co\u017e \u010dasto vede k recidiv\u011b po l\u00e9\u010db\u011b. Permethrin m\u00e1 naproti tomu siln\u011bj\u0161\u00ed \u00fa\u010dinek na huben\u00ed rozto\u010d\u016f a \u010dasto se p\u0159edepisuje jako kr\u00e9m. P\u0159esto\u017ee dok\u00e1\u017ee sn\u00ed\u017eit hustotu populace rozto\u010d\u016f, z\u0159\u00eddkakdy vede k \u00fapln\u00e9mu vym\u00fdcen\u00ed a v n\u011bkter\u00fdch p\u0159\u00edpadech je pozorov\u00e1no pouze \u010d\u00e1ste\u010dn\u00e9 zlep\u0161en\u00ed. P\u0159i opakovan\u00e9m pou\u017e\u00edv\u00e1n\u00ed m\u016f\u017ee tak\u00e9 hrozit riziko sn\u00ed\u017een\u00e9 citlivosti nebo rezistence na l\u00e9k. Pacienti \u010dasto uv\u00e1d\u011bj\u00ed p\u0159etrv\u00e1vaj\u00edc\u00ed sv\u011bd\u011bn\u00ed a z\u00e1n\u011bt, a to i po n\u011bkolika cyklech l\u00e9\u010dby. Ve vy\u0161\u0161\u00edch koncentrac\u00edch m\u016f\u017ee tea tree olej znehybnit a zlikvidovat <i>Demodex<\/i>, proto se n\u011bkdy k hygien\u011b o\u010dn\u00edch v\u00ed\u010dek p\u0159i demodekoze pou\u017e\u00edvaj\u00ed z\u0159ed\u011bn\u00e9 p\u0159\u00edpravky.<\/p>\n<p>Vedlej\u0161\u00ed \u00fa\u010dinky v\u0161ak omezuj\u00ed jeho \u0161irok\u00e9 pou\u017eit\u00ed. Koncentrovan\u00fd \u010dajovn\u00edkov\u00fd olej m\u016f\u017ee zp\u016fsobit p\u00e1len\u00ed, podr\u00e1\u017ed\u011bn\u00ed a dokonce i alergick\u00e9 reakce, tak\u017ee nen\u00ed vhodn\u00fd pro dlouhodob\u00e9 pou\u017e\u00edv\u00e1n\u00ed na citliv\u00fdch m\u00edstech, jako jsou o\u010dn\u00ed v\u00ed\u010dka. Krom\u011b toho se v\u00fdrobky dostupn\u00e9 na trhu li\u0161\u00ed silou a slo\u017een\u00edm, co\u017e sni\u017euje konzistenci v\u00fdsledk\u016f. Po ukon\u010den\u00ed l\u00e9\u010dby je tak\u00e9 \u010dast\u00e1 recidiva. <a href=\"https:\/\/meridian.allenpress.com\/journal-of-parasitology\/article-abstract\/104\/5\/473\/7541\" target=\"_blank\" rel=\"noopener\"><sup>(9)<\/sup><\/a><\/p>\n<h3>Syst\u00e9mov\u00e1 antibiotika a retinoidy<\/h3>\n<p>K l\u00e9\u010db\u011b pacient\u016f s refraktern\u011bj\u0161\u00edmi formami r\u016f\u017eovky nebo nuscinu se pou\u017e\u00edvaj\u00ed peror\u00e1ln\u00ed l\u00e9ky, jako jsou antibiotika nebo retinoidy. Tyto l\u00e9ky p\u016fsob\u00ed sp\u00ed\u0161e na sn\u00ed\u017een\u00ed z\u00e1n\u011btu nebo \u00fapravu produkce ko\u017en\u00edho mazu ne\u017e na p\u0159\u00edm\u00e9 odstran\u011bn\u00ed rozto\u010d\u016f. V\u00fdsledkem je, \u017ee mnoho pacient\u016f hl\u00e1s\u00ed \u010d\u00e1ste\u010dnou \u00falevu, ale nad\u00e1le si nesou zna\u010dn\u00fd po\u010det rozto\u010d\u016f, co\u017e pozd\u011bji vede k recidiv\u00e1m.
