Pri hodnoten\u00ed dostupn\u00fdch \u00fadajov je v\u0161ak d\u00f4le\u017eit\u00e9 poznamena\u0165, \u017ee klinick\u00e9 d\u00f4kazy t\u00fdkaj\u00face sa \u013eud\u00ed zost\u00e1vaj\u00fa obmedzen\u00e9. Hoci bolo publikovan\u00fdch nieko\u013eko \u0161t\u00fadi\u00ed na \u013eu\u010foch, z\u00e1klad\u0148a d\u00f4kazov je v\u00fdrazne men\u0161ia ako pri v\u00e4\u010d\u0161ine konven\u010dn\u00fdch farmaceutick\u00fdch terapi\u00ed.<\/p>\n<p>Jedna z hlavn\u00fdch \u0161t\u00fadi\u00ed na \u013eu\u010foch uskuto\u010dnili Meshchaninov a jeho kolegovia a zah\u0155\u0148ala 32 dospel\u00fdch vo veku od 41 do 83 rokov s chronickou polymorbiditou a remisiou organick\u00e9ho mozgov\u00e9ho syndr\u00f3mu (1). V\u00fdskumn\u00edci hodnotili vplyv Pinealonu a in\u00e9ho bioregula\u010dn\u00e9ho peptidu, Vesugen, na biomarkery biologick\u00e9ho starnutia a ukazovatele funk\u010dn\u00e9ho stavu organizmu.<\/p>\n<p>Vedci zaznamenali, \u017ee Pinealon vyvol\u00e1val v\u00fdznamn\u00e9 anabolick\u00e9 \u00fa\u010dinky a bol sp\u00e1jan\u00fd so zlep\u0161en\u00edm aktivity centr\u00e1lneho nervov\u00e9ho syst\u00e9mu (CNS) a funkci\u00ed nieko\u013ek\u00fdch d\u00f4le\u017eit\u00fdch org\u00e1nov (1). Pod\u013ea autorov \u0161t\u00fadie tieto zmeny zodpovedali spomaleniu ukazovate\u013eov biologick\u00e9ho starnutia. V\u00fdskumn\u00edci dospeli k z\u00e1veru, \u017ee Pinealon vykazuje neuroprotekt\u00edvne aj geroprotekt\u00edvne vlastnosti, ktor\u00e9 m\u00f4\u017eu podporova\u0165 zdrav\u00e9 starnutie a udr\u017eanie funk\u010dnej zdatnosti u star\u0161\u00edch os\u00f4b.<\/p>\n<p>Vy\u0161etranie zah\u0155\u0148alo aj hodnotenie mnoh\u00fdch bunkov\u00fdch a metabolick\u00fdch markerov s\u00favisiacich so starnut\u00edm. Hoci Pinealon vykazoval prospe\u0161n\u00fd vplyv na funk\u010dn\u00e9 v\u00fdsledky, vedci poznamenali, \u017ee jeho geroprotekt\u00edvne \u00fa\u010dinky sa zdali by\u0165 o nie\u010do menej v\u00fdrazn\u00e9 ako tie, ktor\u00e9 boli pozorovan\u00e9 pri Vesugene v tej istej \u0161t\u00fadii (1). Napriek tomu autori pova\u017eovali Pinealon za potenci\u00e1lny neuroprotekt\u00edvny a geroprotekt\u00edvny peptid, ktor\u00fd si zasl\u00fa\u017ei \u010fal\u0161ie hodnotenie u star\u0161ej popul\u00e1cie.<\/p>\n<p>Napriek t\u00fdmto s\u013eubn\u00fdm pozorovaniam je potrebn\u00e9 zv\u00e1\u017ei\u0165 nieko\u013eko obmedzen\u00ed. \u0160t\u00fadia zah\u0155\u0148ala relat\u00edvne mal\u00fd po\u010det \u00fa\u010dastn\u00edkov, nemala mierku, ktor\u00e1 zvy\u010dajne vy\u017eaduje jasn\u00e9 klinick\u00e9 z\u00e1very a zamerala sa na konkr\u00e9tnu skupinu pacientov (1). Na potvrdenie t\u00fdchto v\u00fdsledkov s\u00fa potrebn\u00e9 \u010fal\u0161ie \u0161t\u00fadie zah\u0155\u0148aj\u00face v\u00e4\u010d\u0161ie a rozmanitej\u0161ie popul\u00e1cie.<\/p>\n<p>V s\u00fa\u010dasnosti neexistuj\u00fa rozsiahle, multicentrick\u00e9, randomizovan\u00e9 kontrolovan\u00e9 \u0161t\u00fadie hodnotiace Pinealon na zhor\u0161enie kognit\u00edvnych funkci\u00ed, Alzheimerovu chorobu, zdrav\u00e9 starnutie alebo in\u00e9 klinick\u00e9 indik\u00e1cie. Podobne preh\u013ead\u00e1vanie verejne dostupn\u00fdch datab\u00e1z klinick\u00fdch \u0161t\u00fadi\u00ed poukazuje na relat\u00edvne mal\u00fd po\u010det registrovan\u00fdch \u0161t\u00fadi\u00ed s \u00fa\u010das\u0165ou \u013eud\u00ed v porovnan\u00ed s podrobnej\u0161ie presk\u00faman\u00fdmi terapeutick\u00fdmi zl\u00fa\u010deninami.<\/p>\n<p>Celkovo s\u00fa\u010dasn\u00e9 \u00fadaje z v\u00fdskumu na \u013eu\u010foch nazna\u010duj\u00fa, \u017ee Pinealon m\u00f4\u017ee ovplyvni\u0165 funkciu centr\u00e1lnej nervovej s\u00fastavy a biomarkery s\u00favisiace s biologick\u00fdm starnut\u00edm. Dostupn\u00e9 klinick\u00e9 \u00fadaje v\u0161ak zost\u00e1vaj\u00fa predbe\u017en\u00e9 a s\u00fa potrebn\u00e9 \u010fal\u0161ie, dobre navrhnut\u00e9 klinick\u00e9 \u0161t\u00fadie, k\u00fdm bude mo\u017en\u00e9 vyvodi\u0165 kone\u010dn\u00e9 z\u00e1very o jeho \u00fa\u010dinnosti, bezpe\u010dnosti a terapeutickom potenci\u00e1li u \u013eud\u00ed.<\/p>\n<h2>Bezpe\u010dnos\u0165 a ved\u013eaj\u0161ie \u00fa\u010dinky lieku Pinealon<\/h2>\n<p>Dostupn\u00e9 \u00fadaje nazna\u010duj\u00fa, \u017ee Pinealon je v\u0161eobecne dobre tolerovan\u00fd ako v experiment\u00e1lnych \u0161t\u00fadi\u00e1ch na zvierat\u00e1ch, tak aj v obmedzenom po\u010dte doteraz vykonan\u00fdch \u0161t\u00fadi\u00ed na \u013eu\u010foch. Ke\u010f\u017ee v\u0161ak klinick\u00e9 \u00fadaje na \u013eu\u010foch zost\u00e1vaj\u00fa pomerne obmedzen\u00e9, jeho celkov\u00fd bezpe\u010dnostn\u00fd profil nebol doposia\u013e \u00faplne charakterizovan\u00fd, najm\u00e4 pokia\u013e ide o dlhodob\u00e9 u\u017e\u00edvanie.<\/p>\n<p>Najhodnotnej\u0161ie \u00fadaje o bezpe\u010dnosti poch\u00e1dzaj\u00fa z prieskumu Meshchaninova a kolekt\u00edvu, ktor\u00fd zah\u0155\u0148al 32 dospel\u00fdch vo veku 41 a\u017e 83 rokov s chronickou polymorbiditou a organick\u00fdm mozgov\u00fdm syndr\u00f3mom v remisii (1). V\u00fdskumn\u00edci zaznamenali zlep\u0161enie funkcie centr\u00e1lneho nervov\u00e9ho syst\u00e9mu a biomarkerov biologick\u00e9ho starnutia bez zistenia v\u00fdznamn\u00fdch bezpe\u010dnostn\u00fdch probl\u00e9mov po\u010das obdobia \u0161t\u00fadie. D\u00f4le\u017eit\u00e9 je, \u017ee Pinealon sa nezdal negat\u00edvne ovplyv\u0148ova\u0165 kondenz\u00e1ciu chromat\u00ednu, \u010do viedlo autorov k z\u00e1veru, \u017ee peptid vykazuje bezpe\u010dnos\u0165 na \u00farovni genetickej stability bunkov\u00e9ho jadra za sk\u00faman\u00fdch podmienok (1).<\/p>\n<p>\u0160t\u00fadie na zvierat\u00e1ch a bunk\u00e1ch tie\u017e poukazuj\u00fa na priazniv\u00fd bezpe\u010dnostn\u00fd profil. \u0160t\u00fadie hodnotiace \u00fa\u010dinky Pinealonu na oxida\u010dn\u00fd stres, pre\u017eitie neur\u00f3nov a kognit\u00edvne funkcie zvy\u010dajne preuk\u00e1zali ochrann\u00e9, nie \u0161kodliv\u00e9 \u00fa\u010dinky (2 \u2013 4). V experiment\u00e1lnych modeloch Pinealon zni\u017eoval oxida\u010dn\u00e9 po\u0161kodenie, obmedzoval akumul\u00e1ciu reakt\u00edvnych foriem kysl\u00edka, zlep\u0161oval \u017eivotaschopnos\u0165 neur\u00f3nov a zni\u017eoval nekr\u00f3zu buniek, bez zn\u00e1mok v\u00fdznamnej toxicity pri testovan\u00fdch d\u00e1vkach (2, 3).<\/p>\n<p>Napriek s\u013eubn\u00fdm v\u00fdsledkom je potrebn\u00e9 zoh\u013eadni\u0165 nieko\u013eko d\u00f4le\u017eit\u00fdch obmedzen\u00ed. \u0160t\u00fadie na \u013eu\u010foch s\u00fa st\u00e1le obmedzen\u00e9 po\u010dtom \u00fa\u010dastn\u00edkov a trvan\u00edm, \u010do s\u0165a\u017euje identifik\u00e1ciu zriedkav\u00fdch ne\u017eiaducich \u00fa\u010dinkov a \u00fapln\u00e9 pos\u00fadenie dlhodobej bezpe\u010dnosti. Okrem toho sa zatia\u013e neuskuto\u010dnili rozsiahle randomizovan\u00e9 kontrolovan\u00e9 \u0161t\u00fadie zameran\u00e9 \u0161pecificky na hodnotenie bezpe\u010dnosti.<\/p>\n<p>V d\u00f4sledku toho zost\u00e1va dlhodob\u00fd bezpe\u010dnostn\u00fd profil Pinealon neist\u00fd. Ot\u00e1zky t\u00fdkaj\u00face sa chronick\u00e9ho u\u017e\u00edvania, opakovan\u00fdch lie\u010debn\u00fdch cyklov, potenci\u00e1lnych liekov\u00fdch interakci\u00ed a pou\u017eitia u \u0161pecifick\u00fdch popul\u00e1ci\u00ed neboli dostato\u010dne pre\u0161etren\u00e9 v rozsiahlych klinick\u00fdch \u0161t\u00fadi\u00e1ch. Jedinci s existuj\u00facimi zdravotn\u00fdmi probl\u00e9mami by sa mali poradi\u0165 s kvalifikovan\u00fdm zdravotn\u00edckym pracovn\u00edkom predt\u00fdm, ako zv\u00e1\u017eia pou\u017eitie ak\u00e9hoko\u013evek experiment\u00e1lneho peptidu.<\/p>\n<p>Na z\u00e1klade aktu\u00e1lne dostupn\u00fdch \u00fadajov sa zd\u00e1, \u017ee Pinealon je v podmienkach \u0161t\u00fadie dobre tolerovan\u00fd a v publikovan\u00fdch \u0161t\u00fadi\u00e1ch neboli identifikovan\u00e9 \u017eiadne v\u00fdznamn\u00e9 bezpe\u010dnostn\u00e9 sign\u00e1ly. Obmedzen\u00fd rozsah \u0161t\u00fadi\u00ed zah\u0155\u0148aj\u00facich \u013eud\u00ed v\u0161ak znamen\u00e1, \u017ee pred formulovan\u00edm komplexn\u00fdch z\u00e1verov o bezpe\u010dnosti s\u00fa potrebn\u00e9 \u010fal\u0161ie klinick\u00e9 \u0161t\u00fadie.<\/p>\n<h2>Schv\u00e1lenie Pinealonu agent\u00farou FDA a regula\u010dn\u00fd stav<\/h2>\n<p>Mnoho \u013eud\u00ed, ktor\u00ed h\u013eadaj\u00fa inform\u00e1cie o Pinealone, chce zisti\u0165, \u010di bol schv\u00e1len\u00fd americk\u00fdm \u00daradom pre kontrolu potrav\u00edn a lie\u010div (FDA) a \u010di je v Spojen\u00fdch \u0161t\u00e1toch uzn\u00e1van\u00fd za schv\u00e1len\u00fd liek. Na z\u00e1klade s\u00fa\u010dasn\u00fdch regula\u010dn\u00fdch inform\u00e1ci\u00ed Pinealon nebol schv\u00e1len\u00fd FDA na lie\u010dbu, prevenciu ani diagnostiku akejko\u013evek choroby.<\/p>\n<p>Preh\u013ead\u00e1vanie verejne dostupn\u00fdch datab\u00e1z FDA t\u00fdkaj\u00facich sa liekov neuv\u00e1dza Pinealon ako schv\u00e1len\u00fd liek na predpis. Rovnako sa Pinealon nenach\u00e1dza v datab\u00e1ze Drugs@FDA ako schv\u00e1len\u00fd terapeutick\u00fd produkt. To znamen\u00e1, \u017ee Pinealon nepre\u0161iel rozsiahlym procesom regula\u010dn\u00e9ho pos\u00fadenia po\u017eadovan\u00fdm na z\u00edskanie farmaceutick\u00e9ho schv\u00e1lenia v Spojen\u00fdch \u0161t\u00e1toch.<\/p>\n<p>Namiesto toho je Pinealon vo v\u0161eobecnosti pova\u017eovan\u00fd za experiment\u00e1lny bioregula\u010dn\u00fd peptid alebo v\u00fdskumn\u00fd peptid. V\u00e4\u010d\u0161ina s\u00fa\u010dasn\u00fdch vedeck\u00fdch d\u00f4kazov poch\u00e1dza z laborat\u00f3rnych \u0161t\u00fadi\u00ed, \u0161t\u00fadi\u00ed na zvierat\u00e1ch a obmedzen\u00fdch \u0161t\u00fadi\u00ed na \u013eu\u010foch, ktor\u00e9 sa uskuto\u010dnili hlavne v Rusku a v\u00fdchodnej Eur\u00f3pe (1-4). Aj ke\u010f tieto \u0161t\u00fadie preuk\u00e1zali potenci\u00e1lne prospe\u0161n\u00e9 biologick\u00e9 \u00fa\u010dinky, nie s\u00fa dostato\u010dn\u00e9 na to, aby opodstatnili schv\u00e1lenie FDA alebo ustanovili uznan\u00e9 lek\u00e1rske indik\u00e1cie.<\/p>\n<p>Absencia schv\u00e1lenia FDA automaticky neznamen\u00e1, \u017ee zl\u00fa\u010denina nevykazuje \u017eiadnu biologick\u00fa aktivitu. Namiesto toho to znamen\u00e1, \u017ee v r\u00e1mci regula\u010dn\u00e9ho procesu FDA neboli predlo\u017een\u00e9 a schv\u00e1len\u00e9 dostato\u010dn\u00e9 d\u00f4kazy t\u00fdkaj\u00face sa bezpe\u010dnosti, \u00fa\u010dinnosti, kvality v\u00fdroby a klinick\u00fdch pr\u00ednosov.<\/p>\n<p>V\u00fdskumn\u00edci na\u010falej analyzuj\u00fa Pinealon pre potenci\u00e1lne pou\u017eitie v oblasti neuroprotekcie, kognit\u00edvnych funkci\u00ed, regul\u00e1cie oxida\u010dn\u00e9ho stresu a zdrav\u00e9ho starnutia. Tieto oblasti v\u0161ak zost\u00e1vaj\u00fa experiment\u00e1lne a v s\u00fa\u010dasnosti \u017eiadny v\u00fdznamn\u00fd regula\u010dn\u00fd org\u00e1n Pinealon neuzn\u00e1va ako schv\u00e1len\u00fa lie\u010dbu neurologick\u00fdch por\u00fach, stavov s\u00favisiacich s vekom ani ak\u00fdchko\u013evek in\u00fdch lek\u00e1rskych indik\u00e1ci\u00ed.<\/p>\n<p>Vzh\u013eadom na to by sa Pinealon mal v s\u00fa\u010dasnosti pova\u017eova\u0165 za experiment\u00e1lnu zl\u00fa\u010deninu, nie za schv\u00e1len\u00fd liek. Z\u00e1ujemcovia o Pinealon by si mali uvedomi\u0165, \u017ee tvrdenia o jeho terapeutick\u00fdch \u00fa\u010dinkoch s\u00fa zalo\u017een\u00e9 predov\u0161etk\u00fdm na predklinick\u00fdch \u00fadajoch a obmedzenom po\u010dte \u0161t\u00fadi\u00ed na \u013eu\u010foch, a nie na uzn\u00e1vanom regula\u010dnom schv\u00e1len\u00ed.<\/p>\n<h3>Zrieknutie sa zodpovednosti<\/h3>\n<p>Obsah je \u010disto vzdel\u00e1vac\u00ed a informa\u010dn\u00fd a nemal by sa interpretova\u0165 ako lek\u00e1rska rada, diagn\u00f3za, lie\u010dba ani odborn\u00e9 odpor\u00fa\u010danie.
